This is the sign up form for purchasing ticket(s) to the 43rd Annual Design Excellence Awards Ceremony.

Filling out this form does not garantee admission to the 43rd Annual Design Excellence Awards Ceremony. Additionally, you must also pay the invoice that will be sent from QuickBooks, which will be sent out by end of day or the next business day. In the case of Thursdays, you will receive an invoice on the following Monday as the office is not open on Fridays or weekends. Space is very limited and this is first comes first served. Invoices must be paid within 2 business days of receiving the invoice. The cut off date for ticket sales is May 10th.

 

(Member = $250 per ticket, non-member = $300 per ticket) + tax
$2500 per table + tax. This is for 8 tickets, and guaranteed to have all 8 guests at the same table.
$100 per ticket + tax. The EDU email address of the school, must be used to purchase the ticket.
